Please fill out and bring the intake packet (fill it out for yourself if you're the client; fill it out for your child if they're the client), and the consent to treat a minor form (if your child is our client) to your first appointment.

 

We will go over these forms with you in more detail in-person. The HIPAA Privacy Practice form is for you to keep, as well as the Suicide Prevention Hotline form.
